It’s time for your no-cost annual health assessment!

Keeping you healthy at home is important to us. We partner with health plans to provide comprehensive in-home health visits. This special visit is offered at no additional cost to you. The visit does not replace medical care that you receive through routine office visits, and is not the same as the annual wellness visit offered by your primary care provider.

We offer both in-person and telehealth visit options. During the visit you will get quality time with one of our highly skilled clinicians. The clinician will review your medical history and current medications. You will learn more about your current health, and identify any additional topics for you to discuss with your primary care provider. You will even receive a personalized check list of what to ask at your next primary care visit.

Getting ready for your health assessment visit…

  • Gather all of your medications, including prescriptions, over the counter medications, and supplements
  • Have your medical records, health insurance card, and current lab results available for review.
  • Write down any questions you would like to ask the clinician.

Telehealth visits

Telehealth visits provide a safe, no-contact option for your visit. Your clinician will spend about an hour with you at the virtual visit. To ensure the best visit experience, please follow these tips:

  • Make sure your device is adequately charged for the approximately 1 hour visit.
  • Make sure you have strong internet connection.
  • Have all medications, medical records and lab results close by.
  • If you have a family member who helps manage your health, he or she also may attend.

In-person visits

Our licensed clinician will travel to your home to meet with you, review your medical information, medications, check vitals, and order labs. Your visit will last about an hour. To ensure the best visit experience:

  • Designate a quiet place in your home to conduct your health visit.
  • Have your paperwork and medications ready for review.
  • Make sure pets are secured in a designated area so they don’t interfere with the appointment.
  • If you have a family member who helps manage your health, he or she also may attend.
